Participants in 2010Summer Program - fill out and submit 1)application form, have it signed by parent or guardian if you are under 18 years of age. Bring it along with payment on the first day of the program, or send it in mail to Crescent Boat Club #5 Kelly Drive, Phila. PA 19130.
