|Key Voucher

I, ___________________________________________ have received a front door key to Crescent Boat Club. By accepting this key I agree to the following conditions:

1. Keys are not to be loaned. If a key is needed contact the President of Crescent Boat Club. Anyone found loaning a key must forfeit the key.
2. The key is the property of Crescent Boat Club.
3. I will not duplicate the key.
4. If the key is lost I agree to pay $100 for a replacement.
5. I agree to pay a security deposit of $50. Crescent will reimbursed this fee when the key is returned to the club secretary.
6. I agree to abide by all rules of Crescent Boat Club and if a member will keep my dues current.
7. Should my relationship and/or membership with Crescent be terminated, I will return the key to the club secretary.
8. I am over 18 years of age.

Special Conditions:

I am a coach of ________________________________ and will return the key upon completion of my coaching duties.

I am a school official of _____________________________________ and will return the key when my association with the crew team expirers.

I am the President of the parent’s association of ________________________________ and will return the key when my term is completed.

Each school will be allocated 4 keys to be distributed to coaches, parent association members or school official. Each person who receives a key is to sign this agreement. Keys are nor to be provided to students.
